In-Home Assessment: Shelly or another RN visits to assess your loved one's home, health status, mobility, and specific care needs. We discuss goals, preferences, and any concerns from the family.

Care Plan Development: Based on the assessment, we develop a personalized care plan outlining what support will be provided, the schedule, and how we'll measure success.

Caregiver Introduction: We introduce you to the specific caregiver(s) who will provide care. You'll have time to build trust and communicate preferences before regular care begins.

Ongoing Monitoring: Throughout the engagement, our RN stays involved—monitoring health, adjusting the care plan as needs change, and maintaining communication with your loved one's physicians.
